This era of care demands more than compliance. Because the patient experience varies from hospice to home health to specialty infusion, etc. — and because every patient differs in drug needs, levels of illness, acuity and more — everyone receiving care requires an individualized approach to care.
One way CitusHealth drives unique experiences is through user tags, which allow you to group caregivers or patients by diagnosis, therapy, line type, etc. In this blog, we’ll explore three big ways these user tags can personalize the patient experience.
The CitusHealth Education Center can personalize the patient experience.
By adding user tags to educational materials to drive them to specific users, patients know that what they receive is targeted and applicable to them, their medications and their diagnosis.
Use case: For home health agencies, targeted groups can be tagged for specific illnesses, such as diabetes or congestive heart failure. Educational materials with those tags will only be available to that group of users. This keeps patients and caregivers from getting overwhelmed with too much information. Instead, they are empowered knowing what they’ve received is specific to their needs.
The CitusHealth Message Center can personalize the patient experience.
When sending broadcast messages (one-way messages sent in bulk), user tags help ensure certain communication is only sent to relevant recipients with that same user tag. It can also be used to tag auto-routing for incoming patient messages.
Use case: A study was published on a new drug that treats HIV patients. This study can be tagged for this diagnosis and sent via broadcast message to patients with that user tag. Patients are then only receiving materials relevant to their needs, they feel seen, and they’re more likely to adhere to their care plans.
The CitusHealth Forms Center can personalize the patient experience.
In the Forms Center, you can send customized forms by user tag, allowing you to get the form in front of a specific group of patients.
Use case: You need to send an education series to heart failure patients. When customizing your form, you can add an embedded YouTube link to the series and a user tag for that diagnosis. That form then sends to patients with that same user tag — getting an important, timely study in the hands of patients it could impact.
No two patients are alike. Add in the complexity of diagnosis, drugs and more, and it can create time-consuming processes to communicate with each individual patient while keeping a personalized approach. With the right technology, efficient workflows make it easy to meet these expectations — while improving patient care.
